Background: Pancreatic ductal adenocarcinoma (PDAC) presents challenges in patient management, particularly in identifying candidates for complex arterial resections. Advances in multimodal therapies have improved the safety of pancreatic cancer surgery, with neoadjuvant chemotherapy regimens like modified FOLFIRINOX (mFOLFIRINOX) and nabpaclitaxel/gemcitabine enabling intricate vascular resections. However, arterial resections remain less common, despite potential oncological benefits. Objective: This study aims to analyze and compare outcomes between PDAC patients undergoing vascular resections and those undergoing standard resections, utilizing data from the German Health Research Data Portal (FDPG). Methods: Employing a retrospective cohort design, data will be sourced from the "Forschen für Gesundheit Portal." Eligible patients will include those who underwent surgery with (Group 1) and without (Group 2) vascular resection. The primary endpoint is in-hospital mortality. A comprehensive array of variables, encompassing patient demographics, admission details, diagnostic and procedural information, medication data, and laboratory parameters, will be analyzed. Results: The study will provide insights into the efficacy and safety of vascular resections in PDAC cases, facilitating the identification of patient subsets that could benefit from such complex procedures. By examining a diverse range of variables, the study aims to contribute to the understanding of outcomes associated with vascular resections and their impact on patient survival. Conclusion: This research aspires to enhance the understanding of surgical interventions in PDAC, specifically focusing on the utilization and outcomes of vascular resections. By leveraging the German Health Research Data Portal, the study seeks to provide valuable insights for multidisciplinary healthcare teams, concerning locally advanced PDAC and potentially improving patient outcomes through informed decision-making.
